If you are in need of a Psychological Evaluation, we are happy to help!

Please complete the appropriate New Client Paperwork and review the instructions below.

Once your paperwork is completed a member of our staff will process the request. 

A Client Portal account will be created and you will receive two emails:

1. therapyportal.com - This email will include a single-use activation link to set up your account and complete the necessary paperwork. (Please note: This link is time-sensitive and only valid for 7 days. If it expires, you will need to request a new one.)

2. schedule@scissortailtherapies.com - This email, labeled Scissortail 'Front Desk', will outline the next steps and include PDF files to guide you through the process.

Once all of your paperwork is completed, someone will contact you via email with our first available appointment date and time. For your convenience, new client evaluation intake appointments can be attended either virtually or in our office. However, the testing portion must be completed in-office.

Due to the high demand for therapy and evaluations, our current new client appointments are being scheduled several months out. We work hard to get you in as quickly as possible without compromising care, and we appreciate your patience and understanding.
