If an evaluation was recommended by an attorney, is court-ordered, involves custody or divorce matters, or relates to any ongoing legal case, please complete the Forensic Case Screening & Eligibility Form below. This ensures proper case review and helps prevent potential delays in your case.
If you are seeking an educational evaluation for the purpose of obtaining an IEP or 504 plan, please note that we do not perform formal educational evaluations for diagnosing learning disabilities. However, we can highlight any significant differences between intelligence and academic performance as part of our assessment. *Please be aware that this testing incurs an additional charge and is not covered by insurance.*
Once your paperwork is completed send a text message to 405-256-8230 saying 'paperwork completed' and a member of our staff will process the request and get you scheduled!
For your convenience, new client evaluation intake appointments can be attended either virtually or in our office. However, the testing portion must be completed in-office.
Due to the high demand for therapy and evaluations, our current new client appointments are being scheduled several months out. We work hard to get you in as quickly as possible without compromising care, and we appreciate your patience and understanding.
